Job Title: Home Health RN Case Manager

About the Role:

As a Home Health RN Case Manager at CenterWell Home Health, you will play a vital role in delivering high-quality patient care and improving health outcomes. You will work closely with our interdisciplinary team to provide comprehensive care to patients in their homes, focusing on their unique needs and goals.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to address their specific requirements.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide skilled nursing visits, administering medications, wound care, and other treatments as prescribed by physicians.
  • Monitor patients' progress, adjusting care plans as needed to ensure optimal health outcomes.
  • Communicate regularly with patients, families, and caregivers to educate them on care plans, medication management, and other essential aspects of care.
  • Document patient interactions, progress, and care plans accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.

Requirements:

  • Diploma, Associate, or Bachelor's Degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related setting
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Excellent organizational, communication, and interpersonal skills
  • A valid driver's license, auto insurance, and reliable transportation
