LHC Group is committed to a culture of delivering exceptional care to patients in West Virginia. We are seeking a skilled Home Health Physical Therapist to join our team in serving patients throughout Randolph, Webster, Lewis, Braxton, and Upshur Counties.
About the RoleAs a Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, you will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
The ideal candidate will have current Physical Therapy licensure in the state of West Virginia, current CPR certification, and access to a dependable vehicle. You will also have the ability to build trusted therapist-patient relationships, provide continuing education and tuition reimbursement opportunities, and foster independence and autonomy.
Responsibilities- Evaluate patients' functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
- Observe and report patients' responses to treatment and any changes in their condition to the supervising nurse and/or the physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Inform and instruct participating members of the healthcare team, patients, and their families/caregivers regarding the plan of care, functional limitations, and progress toward goals.
LHC Group offers a competitive salary, ranging from $60,000 to $120,000 per year, based on full-time employment and maximum productivity. Relocation assistance of $5,000 is available for those who qualify for full-time employment.
