Clinical Pharmacist

As a Clinical Pharmacist within MaineHealth's ambulatory pharmacy program, you will play a vital role in our growing Diabetes and Endocrinology program. Your expertise will help provide medication therapy to patients, ensuring they receive the best possible care.
Key Responsibilities- Collaborate with healthcare professionals to provide safe and cost-effective patient care, including participation in collaborative drug therapy management.
- Communicate directly with patients and their families on all aspects related to medication therapy, promoting high levels of knowledge, engagement, and adherence.
- Ensure appropriate and optimized medication therapy for patients with endocrine disorders.
- Provide education to other healthcare professionals, including precepting trainees, to enhance clinical competency on topics related to therapy.
- Partner with other healthcare professionals to develop evidence-based clinical pathways.
- Generate and analyze process and outcome measures for use in assessing medication-related patient outcomes.
- Graduate of an ACCP accredited school/college of pharmacy.
- Eligible or currently licensed in the State of Maine.
- Work experience as a patient care pharmacist involved in patient monitoring and drug therapy review is preferred.
- Excellent verbal and written communication skills are required.
- Analytical skills necessary to identify and resolve problems related to drug therapy.
