Job Title: Registered Nurse

West Side Health Care District is seeking a skilled Registered Nurse to fill the role of Care Coordinator. This position will support clinic operations, including health plan-driven quality programs, HEDIS measurements, and patient education. The successful candidate will work closely with the Care Coordinator Supervisor to implement patient care plans and provide necessary services for patients' medical, behavioral, and social needs.

Responsibilities:
  • Implement patient care plans and provide support for clinic operations
  • Work with the Care Coordinator Supervisor to ensure seamless patient care
  • Provide administrative, clerical, and clinical duties under the direction of a physician
  • Collaborate with healthcare teams to ensure high-quality patient care
Requirements:
  • Active license as a Registered Nurse with at least 2 years of experience in a healthcare setting
  • OR 2 years of experience as a Case Manager in a healthcare setting
  • BLS/CPR Certification
  • Bilingual Spanish/English (healthcare versed)
  • Case Management experience (preferred)
  • Active Driver's License and Proof of state required Auto Liability Insurance
