Role Summary: This position encompasses two primary functions: as a Medical Assistant, you will enhance the operational efficiency of the practice by serving as a crucial link between healthcare providers and patients, ensuring exceptional service and clinical support. As a Chronic Care Specialist, you will be responsible for managing patient enrollment in chronic care programs and providing coverage during provider absences.

Key Responsibilities:

General Duties:
• Uphold patient confidentiality in compliance with HIPAA regulations.
• Execute administrative and clinical tasks as directed by healthcare providers.
• Exhibit outstanding verbal and written communication skills.
• Welcome and assist patients in a timely and professional manner.
• Accurately gather, verify, and enter patient information into the system.
• Respond promptly to nurse calls.

Clinical and Chronic Care Responsibilities:
• Guide patients to and from examination rooms and treatment areas as necessary.
• Document patient information prior to physician consultations, including vital signs and medical history.
• Ensure all required patient records are obtained and consent is verified before treatments.
• Prepare and maintain examination rooms, ensuring they are stocked and sanitized, and that supplies are current.
• Assist physicians during examinations and procedures.
• Facilitate patient education initiatives by distributing educational materials as needed.

Clinical Tasks Include:
• Administering injections and conducting various diagnostic tests.
• Collecting and labeling specimens for laboratory analysis.
• Preparing patients for examinations, including positioning and site preparation.
• Administering medications and treatments as prescribed.

Patient Engagement:
• Conduct monthly counseling sessions for chronic care management patients.
• Maintain accurate records of patient interactions and follow up on prescribed treatments.
• Schedule necessary tests and appointments for patients.

Qualifications:

Education and Experience:
• High School Diploma or equivalent required.
• Associate's Degree in Medical Assisting preferred.
• 1-2 years of relevant experience preferred.
• Bilingual candidates are preferred.

Skills and Competencies:
• Strong communication skills.
• Commitment to confidentiality.
• Proficient in computer applications.
• Detail-oriented with effective time management abilities.
• Ability to work collaboratively in a team environment.
