Job Description
We are seeking a dedicated and compassionate LPN to join our team at SKY Rehab. As an LPN, you will be responsible for providing high-quality patient care, including administering medications, wound care, and vital sign monitoring. You will also be responsible for documenting patient information, educating patients and families, and collaborating with the interdisciplinary team to develop and implement individualized care plans.
Requirements and Qualifications
To be considered for this role, you must have a current and valid license to practice as a LPN in the state of Kentucky, a current BLS certification from a Vibra-approved vendor, and IV certification. You should also have at least one year of clinical experience in a healthcare setting, with a strong focus on medical-surgical experience. Additionally, you should have excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.

Responsibilities
- Provide high-quality patient care, including administering medications, wound care, and vital sign monitoring
- Document patient information, educate patients and families, and collaborate with the interdisciplinary team to develop and implement individualized care plans
- Manage patient care, improve quality of care, establish professional relationships, and utilize professional skills
- Develop clinical expertise per specialty, demonstrate clinical competence, and manage patient care
