We are seeking a highly skilled and motivated Clinical Pharmacist to join our team in a long-term care setting. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities- Evaluate physician medication orders for appropriateness, potential interactions, and route of administration.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
