About the Role:

The Director of Nursing provides clinical oversight and leadership to ensure high-quality patient care services. This role involves implementing employee relations practices, coordinating referrals, screening patient referrals, evaluating agency capabilities, and ensuring the development and implementation of individualized plans of care.

Key Responsibilities:

  • Implement effective employee relations strategies to promote positive partner morale and relationships.
  • Oversee the coordination of referrals with internal and external sources.
  • Screen patient referrals for appropriateness and evaluate agency capabilities to meet clinical needs.
  • Provide oversight for making patient and partner assignments.
  • Ensure the development, implementation, and updates of individualized plans of care.
  • Oversee all aspects of patient care delivery and coordination.
  • Evaluate patient needs continually and honor patients' rights.
  • Participate in surveys and required reports to maintain compliance.
  • Maintain daily operations by timely completion of tasks within the Home Care Home Based (HCHB) system.
  • Utilize reporting capability for key indicators within HCHB and Strategic Healthcare Partner to achieve desired outcomes.

Required Skills and Qualifications:

  • Nursing Degree from an accredited college or university or equivalent RN program.
  • Two years recent experience as an RN.
  • One year experience in home health required.
  • Current and unrestricted Registered Nurse (RN) licensure in state of practice.
  • Current CPR certification.

Salary and Benefits:

We offer a competitive salary of $90,000-$120,000 per year, depending on experience, along with a comprehensive benefits package including progressive benefits, mileage reimbursement, opportunities for career growth, and flexible schedules.
