Position Overview
We are seeking a licensed LPN or RN to fulfill the responsibilities of an MDS Coordinator. Ideal candidates will have received relevant training or completed an MDS training program. We are also open to training motivated individuals who demonstrate a capacity for learning. The successful applicant will be organized, detail-oriented, possess strong assessment skills, and be capable of working independently. This position operates on a Monday through Friday schedule during daytime hours.
Key Responsibilities:
- Manage MDS scheduling and completion for OBRA/PPS/Managed Care in accordance with RAI guidelines.
- Collaborate with the interdisciplinary team to assist with care plan scheduling and completion.
- Assign and update ICD-10 codes based on physician diagnoses upon admission and with each MDS.
- Submit OBRA/PPS MDS Assessments to CMS following Federal Guidelines.
- Oversee completion and certifications/re-certifications for residents receiving Medicare Part A Benefits.
- Monitor MDS documentation and charting requirements to ensure compliance with services provided.
- Provide education to staff regarding MDS processes as necessary.
- Assist in data collection for audits, including State, Medicare, and Managed Care audits.
- Participate in facility and organizational meetings as per policy.
- Achieve proficiency in and apply RAI rules and regulations.
- Familiarize with Quality Measures (QMs) and assist the team in monitoring performance.
- Must hold a valid LPN or RN license in the state of Indiana.
- Candidates should be organized, detail-oriented, and possess excellent assessment and communication skills, with the ability to work both independently and collaboratively.
