Job Summary:

The Occupational Therapist will provide skilled occupational therapy services to clients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans. Responsibilities:
  • Evaluate clients' level of function and develop comprehensive assessments of their needs
  • Develop and implement individualized care plans in consultation with the physician
  • Provide instruction and guidance to clients and their families on therapeutic self-care and creative activities
  • Conduct regular assessments and updates to ensure the effectiveness of occupational therapy intervention
  • Communicate with the physician and other team members to ensure seamless care coordination
Requirements:
  • Graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A.
  • Licensed or registered by the State
  • At least one year of institutional experience and two years of home care experience
