• Provide direct patient care, including administering medications, performing clinical procedures, and documenting patient data.
• Work collaboratively with the healthcare team to develop and implement individualized care plans.
• Conduct thorough assessments of patients' physical and emotional needs, and develop strategies to address these needs.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Participate in ongoing education and training to stay current on best practices and industry developments.

• Current and valid license as an LPN in the state of New York.
• Graduation from an accredited nursing program.
• Current CPR certification.
• Experience in long-term care or geriatric nursing preferred.
