Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an LPN with Tufts Medicine Care at Home, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities

  • Assess and Implement Care

Conduct thorough assessments of patients' physical and psychosocial needs to determine their health status and develop personalized care plans.

Provide Skilled Nursing Care

Deliver high-quality, evidence-based nursing care to patients in their homes, including wound care, medication management, and education on self-care and disease management.

Collaborate with the Care Team

Work closely with our interdisciplinary care team, including RNs, therapists, and social workers, to ensure seamless coordination of care and optimal patient outcomes.

Communicate Effectively

Communicate clearly and effectively with patients, families, and healthcare team members to ensure that all needs are met and that patients receive the care they need.

Document Patient Information

Accurately and thoroughly document patient information, including assessments, interventions, and outcomes, to ensure that all relevant information is available for future care.

Qualifications

  • Active LPN Licensure

Must be licensed as a Licensed Practical Nurse in Massachusetts and/or New Hampshire.

Experience and Education

Minimum of one year of acute nursing experience, with a strong preference for home care nursing experience.

Other Requirements

Must have a valid driver's license, auto insurance, and current CPR certification.
