Job Summary:
We are seeking a skilled Social Worker (MSW) to join our team. The ideal candidate will have a Master's Degree in Social Work from a CSWE accredited School, licensure as required by the state, and at least one year of experience as a Social Worker in a healthcare and/or hospice setting.
About the Role:
As a Social Worker (MSW), you will play a vital role in providing psychosocial support to patients and their families. Your primary function will be to assess patient needs, develop interventions, and provide services in accordance with the established plan of care. You will work closely with the interdisciplinary team to ensure comprehensive care is provided to our patients.
Responsibilities:
* Provide psychosocial support to patients and their families
* Assess patient needs and develop interventions
* Collaborate with the interdisciplinary team to ensure comprehensive care
* Utilize professional training and personal judgment to monitor the psychosocial assessment process
Requirements:
* Master's Degree in Social Work from a CSWE accredited School
* Licensure as required by the state in which the hospice is located
* At least one year of experience as a Social Worker in a healthcare and/or hospice setting
* Strong knowledge of terminally ill patients and their families
* Ability to apply knowledge of hospice and psychosocial dynamics of illness, loss, and death
