Job Description

As a key member of the Colusa Indian Community Council, the Tribal Health Nurse Coordinator plays a vital role in managing the public health needs of the Tribal community. This position involves overseeing a comprehensive set of programs and services that promote education, awareness, and accessibility to specialized healthcare services, as well as collecting data for public health purposes.

The ideal candidate for this position is a compassionate and caring individual who will assist Tribal members with various healthcare needs, including preventive, routine, and emergency care. They will also offer guidance on eligibility requirements, care provisions, exclusions, and medical billing and payment processes within the Tribal Health Program.

This position involves actively engaging with the community through outreach efforts, conducting home visits, and advocating for Tribal members. Collaborating closely with clinic staff and community partners, this position plays a vital role in promoting the health and well-being of the Tribal community while contributing to the development and execution of the tribal public health plan.

Key Responsibilities
  • Coordinates with clinic staff to conduct in-home visits for Tribal members who may require additional support and education;
  • Engages with individuals in their own homes to enhance understanding and encouraging active participation in their health journey;
  • Organizes public health events, to feature interactive workshops, guest speakers and demonstrations, to promote key health topics, provide valuable information, promote health literacy, and encourage health lifestyle choices;
  • Identifies specific healthcare specialties needed within the tribal community and make those available to Tribal members;
  • Creates partnership with local organizations and community leaders to create and promote tribal health journey;
  • Collects initial health assessment to develop and maintain data that will guide tribal public health planning and decision-making;
  • Measures and monitors the effectiveness of the public health programs and services implemented; conducts regular assessments to track health outcomes, assess program participations, and gathers feedback from Tribal members;
  • Maintains a safe, secure, and healthy environment for the Member;
  • Works in conjunction with Medical Doctor (MD), Family Nurse Practitioner (FNP), Registered Nurse (RN) and Licensed Vocational Nurse (LVN) to establish a plan of care for Member and coordinate healthy activities;
  • Participates in Inter-Disciplinary Health care meetings at scheduled intervals when necessary;
  • Educates Member on importance of following medication regime as prescribed by Physician;
  • Transports members and their families to and from medical appointments when needed;
  • Communicates key information and/or abnormal findings regarding Member to MD, FNP, RN, and/or LVN immediately;
  • Maintains absolute confidentiality of all information pertaining to Member and family, and adhere to all HIPAA rules;
  • Provides companionship and stimulation for the Member including reading, walks, etc.;
  • Collects, analyzes, evaluates, and presents clinical management data to internal and external leadership; perform operations analysis with appropriate follow-up action plan and intervention.
  • Establishes planning processes in response to corporate strategy, goals, and objectives; monitors progress and accomplishments on a regular basis; promotes and coordinates case management initiatives and program development through appropriate planning, analysis, and developmental phases;
  • Serves as a resource for physicians, providers, members, and staff; participates on assigned committees; promotes effective communication; acts as liaison to other departments, medical staff, and health plans to facilitate information-sharing, collaborative problem solving, and adequate provision of support services;
  • Identifies high-quality community providers and agencies whose services are relevant to client needs;
  • Provides outreach services to engage and connect clients with the community agencies that will help meet their needs and goals;
  • Facilitates and conducts group learning activities;
  • Participates in required training;
  • Must be able to follow plan of care that has been developed for Member; and
  • May perform other duties commensurate with the functions and level of the position.

Minimum Qualifications:

  • Education: Associate Degree
  • Experience: Three (3) years' experience working in the home health or clinic field

Licensing and Other Qualifications:

  • Active unrestricted California RN licenses. LVN license may be used in lieu of California RN licenses, depending on experience level.
  • Ability to possess a Valid First Aid/CPR/AED Certificate
  • Valid California driver's license
  • Case Management (CCM) Strongly preferred

Supervisorial Duties: None

Additional Knowledge, Skills, and Abilities:

  • Ability to assist Member with personal services such as bathing, skin care, and oral hygiene, grooming and dressing if/when necessary; assist Member in toileting if necessary.
  • Transport Member to the store or pharmacy as needed; give assistance to Member to ambulate if necessary; safely complete transfers such as transferring Member from bed to chair or wheelchair to toilet etc.
  • Travel to client's homes in all weather conditions.
  • Must able to participate in entire orientation process & pass skills tests successfully.
  • Follow personal care and/or homemaking plans of care.
  • Efficient time management.
  • Ability to write/type progress notes legible, read, and able to follow instructions.
  • Promote strong interpersonal relationships and deal with the demands of the position.
  • Able to demonstrate effective oral and written communication skills.
  • Self-motivated with the ability to work independently.
