Job SummaryWe're seeking a Licensed Master Social Worker (LMSW) to join our team of compassionate professionals. As a key member of our patient care team, you'll work closely with patients, families, and caregivers to assess psychosocial, environmental, and financial needs, and develop personalized plans of care.
Responsibilities- Conduct thorough assessments of patients' and families' needs, identifying areas for support and intervention.
- Develop and implement comprehensive plans of care, collaborating with patients, families, and caregivers to ensure seamless care coordination.
- Provide case management and advocacy services, connecting patients with community resources and support services as needed.
- Maintain accurate and detailed documentation of patient interactions, progress, and outcomes, adhering to internal regulatory and professional standards.
- Collaborate with interdisciplinary teams to ensure patient-centered care, addressing social and emotional factors that impact health outcomes.
- Must be a Licensed Master Social Worker (LMSW) in the state of employment.
- Must possess a Master's degree from a school of social work accredited by the Council on Social Work Education.
- Must have at least one year of social work experience in a healthcare setting, with Medicare home health or hospice experience preferred.
- Must be capable of making independent decisions relevant to patient needs and prioritizing multiple tasks of critical importance simultaneously.
