About the Role

We are seeking a skilled Occupational Therapist to join our team in Williamsburg, VA. As a Home Health Occupational Therapist, you will provide essential one-on-one care to patients in their homes, supporting their families and promoting independence.

Key Responsibilities
  • Assess and analyze patient care needs, establishing mutual goals with patients and caregivers for a comprehensive plan of care.
  • Ensure the patient's environment is safe and suitable for the delivery of effective care.
  • Identify therapy diagnosis/patient care problems and interventions, making additions or changes as necessary.
  • Implement services and programs related to Occupational Therapy, ensuring orders are carried out in a professional manner.
  • Document additions and/or changes to the patient plan of care, re-evaluating progress and making changes as necessary.
  • Provide patient and caregiver education relating to diagnosis, medications, infection control, and safety measures.
  • Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.
Requirements
  • Graduate of an Occupational Therapist Education program approved by ACOTE.
  • Current Occupational Therapist licensure in the State and American Heart Association CPR certification.
  • Two (2) years of experience in adult occupational therapy, home health experience preferred.
