About the Role:
We are seeking a PMHNP or Psychiatric PA to join our team in Greensboro and Winston-Salem, NC. This role requires a minimum of 3 years of experience prescribing to a Severe and Persistent Mental Illness (SPMI) population. The position involves traveling to the homes of the people we serve, providing onsite care. Full or Part Time hours available, flexible schedule working 5-10 hours per day.
The Assertive Community Treatment Team Nurse Practitioner will evaluate, diagnose, and prescribe medications for individuals diagnosed with mental disorders. Our goal is to enhance the quality of life through services that enable individuals to achieve their personal dreams and goals, while promoting independence and self-determination.
Responsibilities and Qualifications:
* Evaluate individuals served by the Assertive Community Treatment Team for mental health disorders and make an appropriate DSM diagnosis. Ensure services are delivered in a medically appropriate manner, adhering to service definitions and best practices.
* Develop and implement person-centered plans, taking into account individual needs and preferences. Screen referrals in coordination with other Assertive Community Treatment Team members.
* Prescribe medication when appropriate, under the supervision of a physician or according to established policies and protocols. Request diagnostic lab work when necessary.
* Maintain accurate and timely documentation and records, including Electronic Health Records systems. Provide education to individuals to aid in understanding their diagnosis and recommend treatment. Involve family members and other supports in treatment when appropriate.
* Collaborate, coordinate, and consult with other providers to discuss treatment plans, progress in services, and medical aspects of care.
* Participate in staffing of complex or high-risk individuals with other program staff. Attend and actively participate in meetings and training as required. Maintain certification in agency, state, and federal training requirements.
Education and Certifications
A Master's degree in Nursing is required. Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) is also necessary. A valid driver's license is required for this position.
