Job Summary:
St Croix Hospice is seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a Registered Nurse - Case Manager, you will be responsible for providing high-quality patient care and coordinating services to meet the needs of individuals and families in their homes and communities.
Key Responsibilities:
Conduct comprehensive assessments of patients and families to determine hospice needs
Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions
Provide education and support to patients and families on self-care techniques, medication management, and other health-related topics
Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available
Requirements:
Graduate of an accredited school of nursing with current licensure in the state
One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred
Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
