About the Job:
This role involves preparing and reviewing medical records, monitoring resident activity, and establishing standards and policies. You will also coordinate admissions, transfers, and discharges, and transcribe physician orders. To succeed in this position, you must have excellent communication and organizational skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
• Assess residents’ physical and emotional status through preparation and review of medical records and nurse’s notes
• Implement and monitor resident care plans, ensuring alignment with individual needs and preferences
• Develop and maintain relationships with residents, families, and healthcare providers to ensure comprehensive care
• Communicate effectively with interdisciplinary teams regarding resident status, concerns, and changes in condition

Requirements:
• Current LPN license in good standing and local state licensure
• Strong foundation in nursing principles and practices, with a focus on gerontology
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work flexible schedules, including weekends, holidays, and varying shifts
• Physical stamina to perform job duties, with or without reasonable accommodations

Compensation:
A competitive salary range of $55,000-$65,000 per year, based on experience and location, plus a comprehensive benefits package and opportunities for professional growth.
