Job Overview

This is an exciting opportunity for a highly skilled Nurse Practitioner or Physician Assistant to join our team as a Home Health Hospitalist in Danville, Pennsylvania. About the Role

Our Home Health Hospitalists work closely with patients, family members, and healthcare providers to deliver comprehensive care in the comfort of the patient's home. This unique approach to hospital care enables patients who meet admission criteria to receive high-quality care without the need for hospitalization. As an Advanced Practitioner, you will be responsible for assessing patient needs, identifying opportunities for improvement, and collaborating with interdisciplinary teams to develop and implement effective care plans. Your expertise will also include remote patient monitoring and communication to ensure seamless coordination of care. Key Responsibilities

- Assess patient needs and develop individualized care plans
- Collaborate with interdisciplinary teams to coordinate care and address patient concerns
- Utilize remote patient monitoring and communication tools to ensure timely and effective care
- Work closely with patients, family members, and healthcare providers to educate and empower them to take an active role in their care

Education and Certifications

We require a Master's Degree in Nursing (Nurse Practitioner) for this role. Additionally, you must hold a current license as a Registered Nurse in the State of Pennsylvania and maintain Basic Life Support Certification. Certified Nurse Practitioner status through the American Academy of Nurse Practitioners is also required.
