Job Summary

We are seeking a Registered Nurse Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating patient care and services, ensuring that our patients receive the highest level of care and support.

Key Responsibilities:
  • Coordinate patient care and services, including developing and implementing individualized care plans
  • Conduct thorough assessments of patients' medical and social needs
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care transitions
  • Provide education and support to patients and their families
  • Monitor and evaluate patient outcomes, making adjustments to care plans as needed
Requirements:
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One (1) year experience as a RN
  • Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
