Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Lake Sunapee, NH. As a Physical Therapist, you will be responsible for providing high-quality care to adult and geriatric clients in their homes.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop individualized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Develop and implement appropriate interventions, including manual therapeutic exercises, gait training, and balance activities.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Qualifications:
  • A current VT Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
