Job Summary:

Eden Health is seeking a skilled Registered Nurse to provide high-quality patient care services in a hospice setting. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to deliver compassionate and personalized care.

Key Responsibilities:
  • Provide skilled nursing services to patients in their homes, focusing on pain management, symptom control, and overall well-being.
  • Develop and implement individualized care plans, taking into account the patient's physical, emotional, and spiritual needs.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Conduct regular patient assessments, monitoring progress and making adjustments to care plans as needed.
  • Provide education and support to patients and families, empowering them to make informed decisions about their care.
  • Participate in interdisciplinary team meetings, contributing to the development of comprehensive care plans.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements:
  • Graduate of an accredited nursing program.
  • Current licensure as a Registered Nurse in the state of practice.
  • At least one year of experience in a healthcare setting, preferably in hospice or home health care.
  • Valid driver's license and reliable transportation.
  • Proof of current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
