We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Care team at MJHS. As a LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Implement and evaluate patient care plans in collaboration with the Coordinator of Care Nurse
  • Provide skilled nursing services to patients in their homes, including revisits based on skilled need
  • Document patient visits electronically and complete documentation within 24-48 hours
Requirements
  • NY State LPN (Licensed Practical Nurse) License required
  • Minimum of one year Med/surg experience in a hospital or Longterm Care facility required
  • Drivers License and Car required
  • Bilingual Spanish, Mandarin or Cantonese preferred
