As a Long-Term Care Management Professional, you will play a crucial role in achieving this mission by ensuring accurate and timely completion of MDS forms.

Key Responsibilities
  • You will collect information from medical records, bedside assessments, and staff, resident, and/or family interviews to complete the MDS form.
  • You will participate in daily PPS meetings, weekly Medicare meetings, and monthly meetings to ensure federal billing requirements are met.
  • You will review the Plan of Care at least quarterly and with each Comprehensive Assessment to ensure changes during the quarter are included and updated.
Requirements
  1. A Registered Nurse with current state licensure is required.
  2. Minimum three years of clinical experience in a healthcare setting, preferably long-term care.
  3. Minimum one year of MDS experience.
Compensation and Perks

The estimated salary for this role is $75,000 - $100,000 per year, depending on experience.
