Certified Home Health LPN

Job Summary:
connectRN is seeking a skilled Certified Home Health LPN to join our team. As a Certified Home Health LPN, you will provide high-quality patient care and support to patients in their homes. You will work independently and as part of a dynamic team to deliver exceptional patient outcomes.
Responsibilities:
• Educate patients and their families on proper care techniques, medication administration, and health promotion strategies.
• Communicate effectively with patients, families, and healthcare providers to ensure continuity of care and positive outcomes.
• Participate in ongoing training and professional development opportunities to enhance your skills and knowledge.
• Provide one-on-one skilled nursing care and treatment to patients in their home.
• Assist the registered nurse in assessment of patient's condition and contribute information to aid in planning and evaluating care.
• Demonstrate knowledge of home health and competencies in discipline-specific patient care skills required by the care center to provide patient care.
• Perform comprehensive patient assessments and collaborate with a multidisciplinary care team to develop an individualized plan of care as per physician orders that achieve desired outcomes and goals.
• Check patient's vital signs, Perform wound care, and educate patients and their families about their medications and chronic conditions.
• Provide patient episode management per protocol.
• Complete documentation timely and accurately according to industry standards, conventions, and guidelines, including subsequent visits, or other required documentation.
Requirements:
• Current and unencumbered license as a Licensed Practical Nurse (LPN)
• Strong assessment, critical thinking, and communication skills.
• 1+ year(s) of Certified Home Health experience as an LPN
• Ability to work independently and as part of a dynamic team.
• Comfortability using technical tools and software/EMR for visit documentation
• Current CPR certification.
• Reliable transportation, current driver's license, and car insurance coverage.
• Proof of Immunization/Exemption for Flu and COVID-19.
• TB Test or Chest X-Ray.
