Job Summary

We are seeking a highly skilled Occupational Therapist to join our team at Patient Care in Wayne, NJ. As a key member of our healthcare team, you will provide high-quality care to patients in their homes, promoting independence and improving their overall well-being.

Responsibilities
  • Evaluate patients' functional status and occupational therapy needs, developing personalized plans of care to achieve their goals.
  • Consult with physicians to develop and implement therapy plans, ensuring seamless coordination with other healthcare team members.
  • Provide evidence-based occupational therapy services, observing and recording patients' responses to treatment and reporting changes in their condition.
  • Communicate effectively with patients, families, and healthcare team members, educating them on therapy plans, functional limitations, and progress towards goals.
  • Visit patients in their homes, providing services as needed, and completing progress notes and reports according to LHC policy.
  • Participate in staff conferences and committees, contributing to the development of best practices and quality improvement initiatives.
  • Supervise home health aides and occupational therapy assistants, ensuring high-quality care and adherence to LHC policies and procedures.
Requirements
  • Current Occupational Therapy licensure in the state of New Jersey.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • At least 1 year of experience as an Occupational Therapist, with certification as an Occupational Therapist and registration by the National Board for Certification in Occupational Therapy.
