We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team. As a RN Case Manager, you will play a critical role in the provision and coordination of care for our patients. You will be responsible for preventing adverse events, optimizing health service productivity, and ensuring patient outcomes.

This is a full-time position working 8 AM - 5 PM, Monday to Friday. • Provide direct patient care to acute and chronically ill patients in the comfort of their homes
• Validate patient assessments through explicit documentation
• Help patients maintain their personal autonomy and adapt to new situations
• Use critical thinking skills to ensure patient safety needs are met

• Current RN license
• Emotional intelligence and the ability to show compassion and empathy toward others
• Ability to overcome challenges and defuse conflict
