Job Summary:

The Physical Therapist will be responsible for planning, administering, and evaluating physical therapy treatment programs for patients in their homes. This includes testing and screening patients to assess their physical strengths and limitations, developing and implementing treatment plans, and providing education and training to patients and their families.

Responsibilities:
  • Plan and administer physical therapy treatment programs for patients in their homes
  • Test and screen patients to assess their physical strengths and limitations
  • Develop and implement treatment plans to help patients achieve their maximum potential
  • Provide education and training to patients and their families
  • Document patient progress and communicate with healthcare team members
Requirements:
  • Doctor of Physical Therapy (DPT) degree from an institution accredited by the American Physical Therapy Association (APTA)
  • Current and unrestricted license to practice Physical Therapy in the State of California
  • Current CPR/BLS certification
  • Compliant with state healthcare regulatory requirements
  • Ability to care for patients with different personalities and abilities
  • Ability to assess patient condition and work with treatment plans
  • Minimum of 1 year of recent Physical Therapy experience in an institutional setting
  • Minimum of 1 year of recent Physical Therapy experience with a home health agency
