Home Health Speech Therapist Job Description

Job Summary:

We are seeking a skilled and compassionate Home Health Speech Therapist to join our team at Interim HealthCare. As a Home Health Speech Therapist, you will play a vital role in providing personalized speech therapy services to patients in their homes, helping them to overcome communication, swallowing, and cognitive difficulties.

Key Responsibilities:

  • Conduct thorough assessments of patients' communication, swallowing, and cognitive abilities
  • Develop and implement individualized treatment plans to address patients' specific needs and goals
  • Provide one-on-one therapy sessions to patients in their homes, using evidence-based techniques and strategies
  • Collaborate with other healthcare professionals, including RNs, LPNs, CNAs, HHAs, PTs, and OTs, to ensure comprehensive care and coordination
  • Educate patients and their families on communication strategies, swallowing techniques, and cognitive rehabilitation methods
  • Document patient progress and outcomes, and make recommendations for continued care and support

Requirements:

  • Master's or Doctoral degree in Speech-Language Pathology and active SLP license in Arizona
  • Minimum of one year of experience in speech therapy, preferably in home healthcare
  • CPR certification and valid Arizona fingerprint card
  • Knowledge of state and federal home health regulations and guidelines
  • Excellent clinical judgment, critical thinking skills, and communication skills
