They are tasked with communicating observations and requirements to the Program Manager concerning participant needs and objectives.
Key Responsibilities:
- Contribute to the Individual Service Plan (ISP) by providing relevant insights and information.
- Monitor and document progress towards goals and objectives outlined in the ISP.
- Facilitate opportunities for supported individuals to enhance their independence through structured activities.
- Gradually reduce support as progress is made towards ISP goals and objectives.
- Complete daily written documentation accurately.
- Maintain precise records of the support services rendered.
- Document and propose actions to foster the independence of individuals supported.
- Ensure all relevant certifications, including First Aid and CPR, are up to date.
- Respond to crises in accordance with established policies and training.
- Transport clients as needed using company vehicles.
- Make decisions aligned with agency training and mission.
- Perform other duties as assigned.
Qualifications:
- The ideal candidate is dependable, enthusiastic, professional, adaptable, and capable of fulfilling the listed job responsibilities.
- Exhibit positive role modeling for individuals supported.
- Possess effective communication skills.
- Hold a valid driver's license with a clean driving record.
- Pass all necessary background checks.
- Demonstrate the ability to read and interpret documents, write specific reports and correspondence, apply mathematical concepts to practical situations, solve complex problems, and understand both written and verbal instructions.
Physical Requirements:
- Personal Assistance: Ability to assist individuals with daily living activities and related job tasks.
- Lifting / Physical Demands: Capable of lifting 50 pounds or more frequently.
- Challenging Behaviors: Willingness and ability to support individuals exhibiting challenging behaviors.
- Ability to navigate throughout the facility and community.
- Communicate effectively with supported individuals, staff, and the community.
Education and Experience:
- A high school diploma or equivalent is required.
- Must maintain First Aid and CPR certification.
- Experience working with adults with developmental disabilities or similar experience is highly preferred.
