Key Responsibilities- Delivering nursing care to residents in post-acute care settings
- Ensuring adherence to resident care plans
- Monitoring vital signs and reporting findings to supervisors
- Observing residents' conditions and communicating any concerns to the nursing team
- Assisting with meal service and feeding as necessary
- Supporting residents with daily living activities (ADLs) such as personal hygiene, mobility, and toileting
- Maintaining cleanliness and hygiene in residents' rooms and common areas
- Facilitating communication between medical staff and residents, as well as with visitors
- Assisting with the movement of residents in and out of beds and wheelchairs
- Providing companionship and emotional support to residents
- Current certification as a Patient Care Assistant
- Familiarity with nursing care protocols and medical terminology
- Strong organizational skills and effective communication abilities
- Capability to lift at least 50 lbs
