Key Responsibilities
  • Delivering nursing care to residents in post-acute care settings
  • Ensuring adherence to resident care plans
  • Monitoring vital signs and reporting findings to supervisors
  • Observing residents' conditions and communicating any concerns to the nursing team
  • Assisting with meal service and feeding as necessary
  • Supporting residents with daily living activities (ADLs) such as personal hygiene, mobility, and toileting
  • Maintaining cleanliness and hygiene in residents' rooms and common areas
  • Facilitating communication between medical staff and residents, as well as with visitors
  • Assisting with the movement of residents in and out of beds and wheelchairs
