About the Job:

This Nursing Care Coordinator will be responsible for providing specialized care to specific patient populations, optimizing their care and experience. They will coordinate the plan of care and act as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc.

Responsibilities:

  • Provides hands-on assistance according to department, patient and provider needs, including but not limited to assisting with procedures, performing tests, preparing or administering patient medications, obtaining specimens.
  • Acts as first contact for patients, providing triaging and medical advice where applicable.
  • Coordinates the plan of care and acts as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc.
  • Monitors and manages patient's appointments, testing and labs necessary for the patient population.
  • Assists with obtaining prior-authorizations for the patient population.
