We are seeking an experienced Nurse Practitioner to join our team at Humanity Hospice, LLC. As a key member of our interdisciplinary team, you will provide comprehensive hospice services to patients and their families, working closely with our medical director and other healthcare professionals.
Key Responsibilities- Provide medical care and palliative services to patients with terminal illnesses, in compliance with state and federal regulations.
- Conduct thorough assessments of patients' medical conditions and health histories, ordering and interpreting laboratory and diagnostic tests as needed.
- Develop and implement individualized plans of care, prescribing medications and treatments as necessary, and coordinating hospice care services.
- Complete face-to-face visits with patients to assess eligibility for hospice care, and provide clinical information to our medical director for recertification purposes.
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives and maintain knowledge of current Medicare/Medicaid, state, and federal regulations governing hospice services.
- Master's degree in Nursing with a focus on Nurse Practitioner education.
- Current nursing licensure and CPR certification.
- Minimum one year of experience as a Nurse Practitioner, with a minimum of one year in hospice or palliative care.
- Certification in a specialist area, such as Hospice and Palliative Nursing (CHPN), is preferred.
