Key Responsibilities:
- Develop and implement comprehensive treatment plans in collaboration with the Palliative Medical Director and other healthcare professionals.
- Provide diagnosis, treatment, and follow-up care to patients with complex medical needs.

- Participate in ongoing professional training and education to stay up-to-date on the latest palliative care practices and research.
Requirements:
- Master's degree in Nursing from an accredited program.
- Nurse Practitioner specialty in Family or Adult Medicine preferred.
- Experience with Part B billing and collaborative agreements with supervising physicians.
- ACHPN certification in Hospice and Palliative Care preferred.
