We are seeking a highly motivated and compassionate Senior Support Specialist to join our team at Developmental Services, Inc. - Kokomo.

Job Summary

This is a rewarding opportunity to work as part of a dedicated team providing support services to individuals with Medicaid Waiver needs.

You will be responsible for facilitating the person served in reaching their maximum potential by providing ongoing needed and wanted support.

Responsibilities
  • Provide support to designated individuals in accordance with individualized plans.
  • Document major activities of the person served in daily notes and maintain accurate records of support provided.
  • Utilize personal cell phone or company device as needed for time entry and documentation.
  • Plan and assist in scheduling clients' medical appointments, transporting clients to scheduled appointments, and providing care as needed.
  • Pass or monitor medication administration based on needs of persons served.
  • Ensure services are provided in compliance with all funding sources and governing bodies (CARF, BQIS, State and Federal Law).
  • FOLLOW policies/procedures set forth by the Supported Living Division and the agency.
  • Complete daily, weekly, or monthly tasks including but not limited to purchasing groceries for persons served, transporting persons served to medical appointments and/or procedures, and providing shift coverage.
  • Demonstrate patience in all situations involving persons served and communicate effectively using an even, positive tone of voice.
  • Respond appropriately to persons served behavior via verbal and/or physical intervention based on the needs of the persons served.
  • Maintain confidentiality of all people in compliance with agency policies and HIPAA requirements.
  • Monitor the physical condition of the person's home while ensuring a safe and sound environment.
  • Use medical adaptive equipment including wheelchairs, Hoyer Lifts, Shower Chairs, Blood Sugar Testing Equipment, CPAP, Oxygen, BP, and other required devices.
  • Care for the physical needs of persons served including feeding, hygiene, and toileting, and assist in physically transferring/moving persons served when needed.
  • Provide first aid and physical assistance to the individual as needed.
  • Accept supervision by cooperating with supervisor's directives and accepting constructive feedback from supervisor in an effort to improve job performance.
  • Communicate concerns to appropriate supervisory staff.
  • Work scheduled hours as coordinated with Staff Scheduler, which may include irregular shifts outside standard agency operating hours, including weekends and holidays as necessary.
Requirements

To be successful in this role, you must have:

  1. A high school diploma or GED, CNA preferred.
  2. The ability to obtain CPR and first aid certification annually and perform as necessary.
  3. The ability to obtain CPI certification annually and perform as necessary.
  4. A clear criminal record per Medicaid Waiver regulation requirements.
  5. A valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier preferred.
  6. Reliable transportation that meets state requirements for transporting persons served preferred.
  7. The ability to successfully work in both independent and group settings and effectively communicate during crisis situations.
  8. The ability to dress appropriately as defined by agency and department dress code.
