About the Role

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at HCA Houston Healthcare Kingwood. As a key member of our healthcare team, you will play a vital role in ensuring that our patients receive high-quality, patient-centered care.

Key Responsibilities
  • Facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.
  • Assess patients for transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Actively participate in the collaborative program to review patient activities related to medical necessity of hospitalization, appropriateness of length of stay, timely and appropriate utilization of services rendered, and identification of patient needs related to patient care following discharge from the hospital.
  • Promote quality care and cost-effective outcomes to enhance the physical, psychosocial, and vocational health of individuals.
  • Review, plan, implement, coordinate, and evaluate health-related service options throughout the continuum.
Requirements
  • Associate Degree in Nursing, required
  • Bachelor's Degree in Nursing, preferred
  • 1+ years experience in healthcare
