Key Responsibilities- Provide Medically Necessary Services: Develop and implement treatment plans that maximize the health potential of patients, including ordering and interpreting diagnostic tests, identifying appropriate pharmacologic agents, and developing education plans.
- Collaborate with Healthcare Professionals: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and transitions.
- Build Relationships and Partnerships: Establish relationships with hospital-based palliative care teams, community organizations, and healthcare providers to build a culture of collaboration and clinically integrated care.
- Provide Education and Support: Offer disease management instruction and education to patients and their families, and provide clinical guidance to facility staff on patient care issues.
- Maintain Accurate Records: Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
- Comply with Regulations: Ensure compliance with applicable laws and regulations, including Delegation Protocol Agreements, prescriptive authority, and NP scope of practice.
- Education and Experience: Board-licensed nurse practitioner, licensed to practice in New Jersey (PA license may also be required depending on territory served). Hospice and Palliative Care Certification. Complies with current department of Public Safety and maintain Drug Enforcement Agency registrations. DEA Licensure required.
- Skills and Abilities: Knowledge of the palliative care philosophy of care and the delivery of medical care at home. Ability to understand and follow state regulations regarding practice agreement with physician. Excellent communication skills to effectively deliver information between referral source and the hospice clinical team.
