Key Responsibilities- Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient.
- Coordinate and manage patient care services, including skilled nursing visits, physical therapy, occupational therapy, and home health aide services.
- Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Conduct regular patient evaluations and adjust care plans as needed to ensure the highest level of care.
- Communicate effectively with patients, families, and healthcare providers to ensure that all parties are informed and engaged in the patient's care.
- Participate in interdisciplinary care conferences to discuss patient progress and develop comprehensive care plans.
- Document patient information accurately and thoroughly, including progress notes, care plans, and other relevant documentation.
- Diploma, Associate, or Bachelor Degree in Nursing
- A minimum of one year of nursing experience, preferably in a home health or hospice setting
- Strong clinical skills and experience with patient assessment, care planning, and coordination of care
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
