Job Title: Travel Licensed Clinical Social Worker

Job Summary:
We are seeking a Travel Licensed Clinical Social Worker to join our team at Converdia Health Staffing. As a Travel LCSW, you will work within patient homes and inpatient facilities, providing high-quality care and support to patients and their families.

Responsibilities:
  1. Provide clinical social work services to patients and their families in patient homes and inpatient facilities.
  2. Work with interdisciplinary teams to develop and implement patient care plans.
  3. Conduct assessments and develop treatment plans to address patients' physical, emotional, and social needs.
  4. Provide counseling and support to patients and their families to help them cope with illness, injury, or loss.
  5. Collaborate with healthcare providers to ensure seamless care transitions.

Requirements:
  1. Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Wyoming.
  2. Experience working in a clinical social work setting, preferably in a home health or hospice setting.
  3. Ability to work independently and as part of a team.
  4. Strong communication and interpersonal skills.
  5. Ability to travel to patient homes and inpatient facilities as needed.

Benefits:
  • Guaranteed hours: 36 hours/week
  • Contract weeks: 13
  • Medefis billing
  • Guaranteed hours and contract weeks may vary based on agency contract

Additional Information:
We will not accept a traveler with a permanent address within 100 miles of our hospital. Any traveler confirmed but not starting on time will result in a fee being charged to the agency, per their contract.
