Job Title: RN Case Manager

At Arkansas Hospice, we are seeking a compassionate and skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Job Summary

The RN Case Manager is responsible for overseeing and participating in the provision of appropriate clinical interventions and support to an assigned caseload of hospice patients. This includes coordinating and collaborating with interdisciplinary team members to manage the plan of care, updating the plan of care on an ongoing basis, and facilitating effective communication with patients, families, and other healthcare providers.

Responsibilities
  1. Manage the patient care functions after the initial admission and assignment to a designated RN Case Manager.
  2. Assist in the admission process as requested.
  3. Interpret, when necessary, Arkansas Hospice, Inc. policies and procedures for nursing staff and others to assure understanding and compliance.
  4. Coordinate and collaborate with interdisciplinary team members for the management of the plan of care on an ongoing basis to meet patient/family needs.
  5. Update the Plan of Care on an ongoing basis keeping other team members current regarding the progress of the patient's physical status and any changing needs for care.
  6. Facilitate effective written and verbal communication with patients/families and other healthcare providers.
  7. Maintain communication with attending physician and community organizations concerning resources for assigned patients/families.
  8. Monitor pain and symptom control to assure optimal patient comfort.
  9. Coordinate CNA scheduling needs with Patient Care Coordinator/Area Manager or designee.
  10. Initiate referrals for other hospice related care and services.
  11. Serve as a preceptor for nurses in orientation, students, and trainees.
  12. Review documentation of staff as directed; assure documentation is complete, accurate, and timely according to organizational policies and practice standards.
  13. Participate in interdisciplinary team meetings, case conferences, care planning activities, and other committee meetings as assigned.
  14. Actively participate in Performance Improvement activities.
  15. Participate in community outreach activities to educate the public about Arkansas Hospice, Inc. services.
  16. Coordinate and provide education on hospice and hospice services for patients/families, other team members, and the community.
  17. Rotate on-call duty to provide 24-hour service to patients and families as assigned.
  18. Maintain strict confidentiality at all times.
  19. Utilize and model excellent customer service skills at all times; seek opportunities to assist patients, families and co-workers; demonstrate teamwork and cooperation.
  20. Accept direction and follow instructions from supervisor; seek additional information as needed; work with minimal supervision.
  21. Adhere to all organizational and departmental policies and procedures.
  22. Continually meet organizational standards for attendance and punctuality; notify supervisor in a timely manner when employee will be absent or late for work.
  23. Attend all required meetings and in-services; seek opportunities for additional professional development activities as appropriate.
