Job Summary

Responsibilities
  • Deliver Patient-Centered Care: Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Collaborate with Healthcare Team: Work closely with Registered Nurses, physicians, and other healthcare professionals to develop and implement individualized care plans.
  • Monitor Patient Outcomes: Continuously assess patient progress, identifying areas for improvement and reporting concerns to the Assistant Clinical Manager/Supervisor.
  • Administer Medications: Safely and accurately administer medications, following established protocols and guidelines.
  • Document Patient Information: Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
Requirements
  • Education: Completion of an accredited practical nursing program.
  • Experience: A minimum of one (1) year of experience in a Long Term Care Facility is required.
