Sunrise Senior Living is seeking a highly skilled and compassionate LPN Medication Care Manager to join our team. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care and services for our residents. Your primary responsibility will be to administer medications and treatments in a safe and organized manner, while also providing resident care and promoting dignity and physical safety.
Responsibilities:
- Administer medications and treatments to residents in a safe and organized manner
- Document medication administration and ensure accurate and timely documentation
- Conduct Service and Health Updates as directed by the Resident Care Director
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates
- Transcribe orders and maintain accurate and up-to-date records
- Partner with the community team to ensure compliance with OSHA and Occupational Health and Safety Regulation requirements
- Report all accidents, incidents, and unsafe conditions to the Resident Care Director or Wellness Nurse
- Maintain confidentiality of resident information and adhere to Sunrise's Principles of Service
- Assist in checking medication and restocking medication carts
- Count narcotics with another Medication Care Manager or Lead Care Manager each shift
- Follow re-fill process for medications and maintain a clean and organized medication room
- Help residents maintain independence and promote dignity and physical safety
- Strive to understand and respond to each resident's unique communication patterns, history, and basic human needs
- Audit medication carts and maintain accurate and up-to-date records
Qualifications:
- LPN, LVN, or state/province specific licensed nurse credential
- Maintain certifications and ongoing training and re-education as required by Sunrise and state/provincial regulations
- CPR and First Aid certification
- Ability to make choices and decisions and act in the resident's best interest
- Ability to react and remain calm in difficult situations
- Ability to handle multiple priorities and possess written and verbal skills for effective communication
- Demonstrate good judgment, problem-solving, and decision-making skills
