The Patient Care Manager - Hospice collaborates with the Executive Administrator, Executive Director, and Director of Clinical Operations to oversee all patient care activities, ensuring the provision of safe and effective care that complies with established nursing standards, regulatory guidelines, and payer requirements.
- Accountable for adherence to hospice policies and procedures, as well as local, state, and federal regulations governing the practice of the assigned team.
- Proactively promotes and guides the assigned team in maintaining high-quality care and patient safety.
- Engages in comprehensive problem-solving and complaint resolution.
- Conducts training sessions for staff to enhance patient health and safety, familiarize them with regulatory matters, and address any identified educational needs.
- A minimum of 4 years of full-time experience as a Registered Nurse or equivalent is required.
Licensure Requirements
- Current RN licensure in the state of practice with at least one year of clinical experience.
- Current CPR Certification.
- Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
Additional State-Specific Requirements
- Experience with terminally ill patients is required.
