Job Summary:The successful candidate will have a strong background in family medicine, excellent communication skills, and the ability to work independently in a fast-paced environment. You will be responsible for conducting health risk assessments, documenting patient information, and collaborating with other healthcare professionals to ensure the best possible outcomes for our clients.
Responsibilities:- Conduct health risk assessments and document patient information in the EMR system
- Collaborate with other healthcare professionals to ensure the best possible outcomes for our clients
- Provide education to patients and their families on chronic medical conditions, preventative care, and medication adherence compliance
- Complete referrals to case management, nursing, pharmacy, etc. as needed
- Participate in clinical meetings and compliance training
- Perform other duties as assigned by the supervising physician
- Training as a Family NP is required
- Candidates must be board certified by the ANCC or AANP
- Must have active/unrestricted license in the state of position opening
- New graduates will be considered
- Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies
The Family Nurse Practitioner will work in a fast-paced environment, interacting with patients, coders, schedulers, and supervising physicians. The provider will occasionally need to travel to patient homes within the Bronx and Manhattan Counties (NY).
