About the Job:

The Intake Coordinator plays a pivotal role in the patient journey, serving as the single point of entry for in-patient intake. Key responsibilities include phone triage, addressing requests for services, and performing patient call backs, while documenting all call activities. This role requires exceptional communication skills to answer inquiry calls about hospital services and bed availability, as well as pre-registration, data collection, and insurance verification. The Intake Coordinator will also facilitate and coordinate assessment and/or intake to facility services when appropriate, maintaining seamless communication between the organization and patient referrals and family.

Roles and Responsibilities:
  • Conducts pre-admission screenings and schedules assessments.
  • Collaborates with business office, nursing, and clinical staff, as well as external parties, to ensure timely patient admissions.
  • Refers inquiries to other agencies and community resources when not suitable for facility assistance or admission.
  • Provides information and referral services for internal and external customers.
  • Presents clinical information to a member of the medical staff for approval of intake.
  • Offers crisis intervention services when needed.
  • Verifies insurance upon intake.
  • Maintains all documentation involved with the admissions process.
  • Provides ongoing communications with referral sources regarding patient status and promotes available services.
  • Offers clerical support to admissions, including collating admission folders, ensuring supply, auditing patient charts, and monitoring patients in the internal waiting room.
Education, Experience, and Skill Requirements:
  • High school diploma or equivalent required; a Bachelor's degree in social work, psychology, counseling, or a related field is preferred.
  • One or more years of experience in healthcare admissions, preferably in mental health or substance use disorder, is required.
  • Experience in patient assessment, referral, treatment planning, and communication with external review organizations or comparable entities is essential.
  • Knowledge of community resources is necessary.
  • Outstanding interpersonal and interviewing skills, as well as telephone etiquette and paging procedures, are required.
Licenses, Designations, and Certifications:
  • CPR and de-escalation and restraint certification is required (training available upon hire and offered by the facility).
  • First aid may be required based on state or facility requirements.
Supervisory Requirements:This position is an Individual Contributor.
