Responsibilities

  • Provide routine care and support to residents, including assistance with daily living activities, grooming, and mobility.
  • Review patient care plans daily to ensure that care is being delivered in accordance with individual needs and preferences.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
  • Participate in facility surveys and maintain confidentiality of resident care information in accordance with HIPAA guidelines.
  • Assist with transporting residents to/from appointments and activities, and ensure their rooms are ready for receiving.

Qualifications

  • Current certification as a Certified Nursing Assistant in this state.
  • High school diploma or equivalent required.
  • Passion for caregiving and commitment to delivering exceptional patient care.
