**Job Description:**

We are seeking a skilled Hospice Registered Nurse / RN Case Manager to join our team. This role will involve providing direct patient care services to hospice patients and supportive care to their families. The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.

**Responsibilities:**

  • Provide direct patient care services to hospice patients and supportive care to their families.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
  • Collaborate with other healthcare professionals to ensure seamless coordination of care.

**Requirements:**

  • Bachelor's degree in Nursing (BSN) or Master's degree in Nursing (MSN).
  • RN license in good standing in the state where employment is sought.
  • At least one year of experience in hospice or palliative care nursing.
  • Strong communication and interpersonal skills.
