Job Summary

Key Responsibilities

  • Provide geriatric house calls to older adults, delivering evidence-based therapy and promoting independence and quality of life.
  • Develop and implement individualized treatment plans, tailored to the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Participate in ongoing education and professional development, staying up-to-date on the latest research and best practices in geriatric care.

Requirements

  • A valid Speech Language Pathology license in the state(s) of practice, or eligibility to apply.
  • A current Certificate of Clinical Competence (ASHA CCC).
  • A degree from an accredited Speech Language Pathology program.
  • Basic computer literacy skills and proficiency in electronic medical records.
  • Current CPR certification and ability to wear personal protective equipment (PPE).
  • Integrity, compassion, and enthusiasm for delivering exceptional patient care.
