Position Overview:
The Clinical Care Coordinator is responsible for delivering exceptional care to residents while ensuring compliance with all relevant laws, regulations, and standards. This role reports directly to the Director of Nursing.
Qualifications:
Associate's Degree from an accredited institution.
Licenses and Certifications:
- Valid nursing diploma or an Associate's/Bachelor's degree in nursing.
- Current nursing license in the state of Ohio.
- CPR certification required upon hire or during orientation, with ongoing maintenance of certification.
Key Competencies:
- In-depth knowledge of nursing practices and regulations applicable to long-term care facilities.
- Ability to make informed decisions independently when necessary.
- Strong leadership skills with a collaborative approach to supervising both professional and non-professional staff.
- Capable of planning, organizing, and implementing nursing department objectives and policies.
- Familiarity with company policies, including those related to workplace conduct and complaint resolution.
- Proficient in reading, writing, and communicating effectively in English.
- Competent in delivering fundamental nursing care to residents.
- Skilled in assessing resident needs and adjusting care plans as required.
- Ability to provide clinical oversight to LPNs and CNAs.
- Timely and accurate documentation of resident care.
- Dependable attendance and strong reasoning and judgment skills.
- Effective communication and collaboration within an interdisciplinary team.
- Monitoring and reporting changes in residents' health status.
- Maintaining precise and comprehensive records.
- Interpreting diagnostic tests to evaluate resident conditions.
- Modifying treatment plans based on resident responses.
- Collaborating with healthcare team members to assess and implement care plans.
- Overseeing all aspects of resident care, including dietary and physical activity considerations.
- Educating residents and families on health topics and disease prevention.
- Assisting residents during examinations and treatments.
- Preparing clinical environments and ensuring adequate supplies.
- Supporting resident evacuation procedures when necessary.
